Male fertility

Men can experience fertility issues, and they can often be easily corrected. Problems with male fertility can sometimes be caused by low sperm count, low sperm motility or problems related to sperm morphology.

The causes are varied, however they can be broadly divided into three categories:

Lifestyle choices can also impact male fertility, such as smoking, poor diet and lack of exercise. It is also possible for issues to be related to genetics or caused by trauma or radiation.

Issues that can affect the sperm include:

We at Medicover Fertility believe that a thorough examination and exact diagnosis by our clinician is a must for framing a treatment plan to resolve fertility issues. This diagnosis can involve two to three semen analyses, blood tests for a hormone check, the review of the male’s family history and a physical examination.

Treatment is based entirely on the individual and it can range from the most basic drugs to minor surgical procedures to extract sperm. There are many surgical sperm retrieval techniques available that can be useful for such patients.

Percutaneous Epididymal Sperm Aspiration (PESA)

PESA stands for percutaneous epididymal sperm aspiration. After mild sedation, a small needle is inserted through the skin of the scrotum to collect sperm from the epididymis over the testes, where sperm are usually stored after production. The sperm may be retrieved by delicate suction. This method is typically used if the male is not able to produce any sperm during the ejaculation (discharge). The reason for the absence of sperm may be because of some kind of blockage or result of prior surgery.

Testicular Sperm Aspiration (TESA)

TESA is a technique performed for men who experience low sperm count. It is performed under sedation in an operating room. A needle is inserted in the testicle and sperm are extracted from the tissue. TESA is often performed for men with obstructive azoospermia.

Micro-Dissection TESE (Micro-TESE)

Microsurgical TESE (Micro-TESE) is a surgical procedure used to find sperm in men with non-obstructive azoospermia or extreme testicular disappointment. Micro-TESE is particularly useful in these men, as there might be exceptionally limited territories of sperm creation that might be missed during needle biopsies. This includes an operation performed through a small midline entry point in the scrotum, through which one or both testicles can be seen. The specialist looks at the testicle(s) under the magnifying instrument for healthy areas of seminiferous tubules. The surgeon extracts these tissues which are analysed and processed in ART laboratory for the presence of sperms.

Vasectomy Reversal

Vasectomy reversal is a microsurgical operation that reconnects the male reproductive tract / tubes (Vas deferens) that were cut during the vasectomy. The procedure completes the pathway for the sperm to get into the reproductive system. The success of the vasectomy reversal is typically dependent on the amount of time that has passed since the vasectomy. These surgeries are performed under a high magnifying instrument ("microsurgery"). Once the tubes are re-joined, sperm can again course through the urethra.
